We'd encourage you to compose your answers to the longer questions in a word processor and to cut and paste the resulting text into this form. This gives you access to conveniences like spell checking as well as the opportunity to save and reflect on your work before submitting.
(Please Note: Your email address is what we use to track your application and participation in the workshop. Be sure to use the same email address in all forms relating to your workshop participation. This email will also be our primary means of communication with you.)
We encourage all participants to design a poster, no larger than 3’ wide x 4’ tall, that highlights some aspect of your teaching that you would like to share with others. If printing this poster causes a financial hardship, please contact the conveners. You will be given an opportunity to present the poster to the group, and the posters will remain on display throughout the workshop.
To best design the workshop, we need to know a little about your teaching style, and what you hope to get out of the workshop.
Below is a list of the different types of data available through the VEPP website. Gaining a better understanding of your experience and knowledge with these techniques will help us in planning the workshop.
Briefly describe your familiarity with the following data types:

You should plan to arrive in Hilo, HI, no later than 5 PM on Sunday, July 25. Departures should be scheduled no earlier than 7 PM on Friday, July 30 (Saturday morning departures may be necessary for some participants). Transportation from the airport will be provided on the afternoon of Sunday, July 25, and to the airport on the evening of Friday, July 30, and the morning of Saturday, July 31. If you wish, you may rent a vehicle at your own expense. You are welcome to arrive in Hawaii earlier or stay later, and bring family or friends, but we cannot support food/lodging costs outside of the dates listed above, and we cannot house non-workshop attendees. If you wish to stay off-site, you are welcome to make your own lodging arrangements, but you will have to provide your own transportation to and from the workshop each day.
In confirming my registration for this workshop, I agree that any audio recording or images (still or video) made of me during the workshop may be used on the project website, in printed project publications, and in materials developed by the project. I further agree that my contributions to creative works that emerge from workshop activities (e.g., discussions on the email list, contributions to the web page, summaries of group discussion, etc.) may be used by the workshop series, PIs, and conveners for development of future workshop materials, for scholarly reports, and for promotion of the workshop series. Attribution for original contributions will be made whenever possible.
I also accept responsibility to cover any costs incurred should I withdraw from participation in the workshop after June 25, 2010. I understand that arrangements have been made on my behalf that have associated real costs (e.g. logistics, materials), and in many cases, my early confirmation has secured a place that has been denied to other interested colleagues. Exceptions will only be made in cases of extreme personal hardship.
